Reapply for a driving licence following a medical condition
Reapply for your licence if it was revoked/surrendered following a medical condition
1. How to reapply
DVLA will send you a letter when your licence is taken away or surrendered, or if your application for a driving licence is refused. This tells you if there's a period of time you need to wait before getting a new licence.
You can then reapply 8 weeks before the end of this period.
Check with your doctor that you can meet the medical standards for driving before reapplying for your licence.
Car or motorcycle licence
To reapply, fill in a D1 application form and the form for your medical condition, and send them to DVLA.
Select your medical condition to find the right form.
Order a D1 pack, which has everything you need to make an application.
You may need to send evidence of your fitness to drive. The letter from DVLA will tell you if this is the case.
D1 packs are also available from Post Office branches.
Bus, coach or lorry licence
Fill in a D2 application form and the form for your medical condition, and send them to DVLA.
Select your medical condition to find the right form.
Order a D2 pack which has everything you need to make an application.
If you’re aged 45 to 65 you also have to fill in a D4 and send it to DVLA if you haven't done so in the past 5 years. Send it in with your D2 form. Read the information leaflet to help you with filling in the D4.
You may need to send evidence of your fitness to drive. The letter from DVLA will tell you if this is the case.
From the age of 65, your licence will need to be renewed each year and your application must be accompanied by a completed D4 medical report form.
Driving licences issued from 14 October 2012 look different, but existing licences are still valid.
Where to send your application
Send your completed application to DVLA by fax on 0845 850 0095 or by post. Find out about call charges.

2. When you can start driving again
There are different rules for when you can drive again depending on if your licence was voluntarily surrendered, or if it was revoked or refused for medical reasons.
Licences revoked or refused for medical reasons
You must wait for DVLA to tell you that you can start driving again.
Voluntarily surrendered licences
You can start driving again as soon as DVLA receives your application, so long as all of the following apply:
- you are not currently disqualified
- you only drive under the conditions of the licence
- you have the support of your GP or your consultant to continue driving
- your application is less than a year old
- you previously held a licence (GB/EU/Designated)
- you were not disqualified as a high risk offender on or after 1 June 2013
